MDT
E4 Primary Care Network (PCN) holds Multi-Disciplinary Team Meeting every month to discuss our clinically vulnerable patient cohorts, the team’s consist of a wide skillset and knowledge from primary and secondary care to provide the best possible care to the patients of the PCN.
The PCN MDT Meeting allows for an opportunity for patient cases to be discussed with a specialist team with the outcome of a personalised care plan taking into consideration the patient’s physical, psychological, social needs and any other wishes.
The MDT consists of (not limited to): Practice’s General Practitioners, Practice Nurses, PCN Clinical Pharmacist, PCN First Contact Physiotherapist, PCN Social Prescribers, PCN Care Coordinators, Adult Social Care, Geriatrician and Mental Health Practitioners.
